If you are a member of the Settlement Class, you can submit a claim for the following Settlement Class Member Benefits:

Cash Payment A – Documented Losses: You may submit a Claim Form and provide reasonable documentation for losses traceable to the Data Incident for up to $5,000 per Settlement Class Member; OR

Cash Payment B – Alternate Cash: Instead of Cash Payment A, without providing documentation, you may submit a Claim Form to receive a pro rata (a legal term meaning equal share) cash payment in the estimated amount of $50; AND

Medical Records Monitoring: In addition to Cash Payment A (Documented Losses) or Cash Payment B (Alternate Cash), you may also submit a Claim Form to receive one year of free Medical Records Monitoring.

Your Cash Payment will be subject to pro rata increase or decrease depending upon the total value of all Valid Claims submitted.

You must submit a timely and valid claim by September 28, 2026, to receive any Settlement Class Member Benefits. If you fail to submit your Claim Form by the deadline, your claim will be rejected, and you will be deemed to have waived all rights to receive Settlement Class Benefits under the Settlement.
